663dea4a60 Kernel: Return real handler purpose when registered to spurious handler
If we registered a real interrupt handler to a spurious one, we should
return its purpose instead of the spurious purpose string.

fe761e7412 Kernel: Handle real IRQs from line 7 when using the PIC
If we have a real IRQ7 to handle, let's make sure the handler knows to
check if it really occured for this value and not only for IRQ15.

32d9534c67 Kernel: Fix GenericInterruptHandler problems with virtual functions
Because registering and unregistering interrupt handlers triggers
calls to virtual functions, we can't do this in the constructor
and destructor.

5d180d1f99 Everywhere: Rename ASSERT => VERIFY
(...and ASSERT_NOT_REACHED => VERIFY_NOT_REACHED)

Since all of these checks are done in release builds as well,
let's rename them to VERIFY to prevent confusion, as everyone is
used to assertions being compiled out in release.

We can introduce a new ASSERT macro that is specifically for debug
checks, but I'm doing this wholesale conversion first since we've
accumulated thousands of these already, and it's not immediately
obvious which ones are suitable for ASSERT.

39c1783387 Kernel: Allow to install a real IRQ handler on a spurious one
IRQ 7 and 15 on the PIC architecture are used for spurious interrupts.
IRQ 7 could also be used for LPT connection, and IRQ 15 can be used for
the secondary IDE channel. Therefore, we need to allow to install a
real IRQ handler and check if a real IRQ was asserted. If so, we handle
them in the usual way.

A note on this fix - unregistering or registering a new IRQ handler
after we already registered one in the spurious interrupt handler is
not supported yet.

f86be46c98 Kernel: Abstract IRQ controller handling from Interrupt handlers
Now we don't send raw numbers, but we let the IRQController object to
figure out the correct IRQ number.
This helps in a situation when we have 2 or more IOAPICs, so if IOAPIC
1 is assigned for IRQs 0-23 and IOAPIC 2 is assigned for IRQs 24-47,
if an IRQHandler of IRQ 25 invokes disable() for example, it will call
his responsible IRQController (IOAPIC 2), and the IRQController will
subtract the IRQ number with his assigned offset, and the result is that
the second redirection entry in IOAPIC 2 will be masked.

666990fbcb Kernel: Correct Spurious Interrupt handlers' controller model() method
We don't return blindly the IRQ controller's model(), if the Spurious
IRQ handler is installed in IOAPIC environment, it's misleading to
return "IOAPIC" string since IOAPIC doesn't really handle Spurious
IRQs, therefore we return a "" string.

0fc60e41dd Kernel: Use klog() instead of kprintf()
Also, duplicate data in dbg() and klog() calls were removed.
In addition, leakage of virtual address to kernel log is prevented.
This is done by replacing kprintf() calls to dbg() calls with the
leaked data instead.
Also, other kprintf() calls were replaced with klog().

6f914ed0a4 Kernel: Simplify interrupt management
The IRQController object is RefCounted, and is shared between the
InterruptManagement class & IRQ handlers' classes.

IRQHandler, SharedIRQHandler & SpuriousInterruptHandler classes
use a responsible IRQ controller directly instead of calling
InterruptManagement for disable(), enable() or eoi().

Also, the initialization process of InterruptManagement is
simplified, so it doesn't rely on an ACPI parser to be initialized.
