0b79abf5b2 Added new, simpler, linear boot process
Background:
- Ghosts existing boot process is split across multiple files, has affordances for outdated ways of running Ghost and is generally non-linear making it nigh-impossible to follow
- The web of dependencies that are loaded on boot are also impossible to unpick, which makes it really hard to decouple Ghost
- With 4.0 we want to introduce a new, linear, simpler, clearer way to boot up Ghost to unlock decoupling Ghost into much smaller pieces

This commit:
- adds a new ghost.js file which switches between boot mode with `node index` or `node index old` so that if we find bugs we can work around them this week
   - Note: the old boot process will go away very soon, but ghost.js will remain as the interface between the command to start Ghost and the application code
- reworks the database migration process into a standalone utility, so that the DB is handled as one simple step of the boot process, decoupled from everything else
- is missing tests for this new db utility
- leaves a lot of work to do around loading core code, services, express apps in a sensible order, as work to fix this would start to break the old boot process
- doesn't use the new maintenance app because we aren't restarting the server here, instead we have the concept of a "core app" that starts in maintenance mode - need to think about how apps will be decoupled in the near future

Updated head to only output custom favicons (#11949)
closes #11932

- as per the issue, there is no need to output a <link> tag if the favicon is the default /favicon.ico, as all browsers automatically check for this 
- instead the favicon <link> is only output if a custom favicon has been set, telling the browser to look somewhere different to its default location
- some of the tests expected 3 links in the html head, but through the favicon change there are often only 2.

- Loading the admin prior to a build results in: Failed to lookup view "error-404" in views directory
- This fixes that error, by splitting the HTMLErrorRenderer and the ThemeErrorRenderer into two separate things

8680099765 🎨 gscan 1.1.0 & optimisations

- differentiate between errors and fatal errors
- use gscan errors in theme middleware
- Adds a new `error()` method to `currentActiveTheme` constructor which will return the errors we receive from gscan
- In middleware, if a theme couldn't be activated because it's invalid, we'll fetch the erros and send them to our error handler. We also use a new property `hideStack` to control, if the stack (in dev mode and if available) should be shown or the gscan errors (in prod mode, or in dev if no stack error)
- In our error handler we use this conditional to send a new property `gscan` to our error theme
- In `error.hbs` we'll iterate through possible `gscan` error objects and render them.
- remove stack printing
- stack for theme developers in development mode doesn't make sense
- stack in production doesn't make sense
- the stack is usually hard to read
- if you are developer you can read the error stack on the server log
- utils.packages: transform native error into Ghost error
- use `onlyFatalErrors` for gscan format and differeniate fatal errors vo.2
- optimise bootstrap error handling
- transform theme is missing into an error
- add new translation key
- show html tags for error.hbs template: rule

🔥 Remove ghost=true concept from asset url helper

 💯 Introduce CSS minification with cssnano
- add new grunt-cssnano dependency
- wire up grunt task to minify public/ghost.css

🎨 Rename minification config & hash params
- Change minifyInProduction -> hasMinFile
  - this means this asset should have a .min file available
- Change minifyAssets -> useMinFiles
  - this means that in this env we want to serve .min files if available

🎨 Update public/ghost.css to serve .min for prod
- add the new `hasMinFile` property

🎨 Move minified asset handling to asset_url util
- this logic should be in the util, not the asset helper
- updated tests

📖 Error handler always needs asset helper
- this removes the TODO and adds a more sensible comment
- we also need to update our theme documentation around error templates

🔥 Don't use asset helper in ghost head
- use getAssetUrl util instead!
- removed TODO

📖 Update proxy docs
🎨 Simplify asset helper & add tests
- this refactor is a step prior to moving this from metadata to being a url util
- needed to skip some new tests

83f084608f 💁🏻 Moveshared/ to server/public (#8273)

Instead of serving our shared assets from a `shared/` folder, we move the file, which are used server side to `server/public`.
Adds a new `config.paths` entry: `publicFilePath` and renames the middleware to serve the files to reflect the changes.
Adds `404-ghost.png` images to be used by the server side rendered default template `error.hbs`.

This is a replacement PR of #8217 (thanks @TienSFU25 for the whole work 🤗), because these changes are needed urgently and blocking other work.

Adds a new `ghost.css` file in `/core/shared/` to be used for server side template rendering (`error.hbs`, `subscribe.hbs` and `private.hbs`).

27ee1dc7b8 Implement custom errors 2.0 (#8148)

- add a new view type of defaultViews, as this is NOTHING to do with the admin!
- rename user-error.hbs to error.hbs, because this can be for any sort of error
- reimplement custom errors, but with a stack like channels & single templates
- change ghost_head to only not output on 500+ server errors, rather than 400+ user errors
- add coverage for the new template functions

61a8845ab5 Switch to new native system font stack (#7219)

Based on an increasingly popular trend and modern web typography capabilities, switch out Google Fonts for default native system fonts, tailored in a stack to suit every device. Also makes some very minor visual adjustments to suit.

Nixes all references to Google Fonts, and provides a faster rendering experience and fewer http requests. 💃

- Changed password.hbs to private.hbs to match the route name
- added a new input_password helper for rendering the password input with the correct properties
- removed the forward input as this can be handled via urls only

- minor update to text next to the password in settings

7c4381c812 Dynamic Titles in Ghost Admin
- Every route can set a title token that is combined with the blog’s
title, resulting in titles like ‘Content - Test Blog’.
- Subroutes are supported (‘Settings - General - Test Blog’)
- The blog’s name is applied to and taken from the `config` object to
spare Ember a REST call via `store.find(‘settings’)`.
- Tests have been changed to test for the new titles.
- The initially proposed solution
(https://github.com/paddle8/ember-document-title) doesn’t play nice
with EAK, which is why I went with this solution

3583515e44 adding config flags to control all items mentioned in PRIVACY.md

- in config.js, the `privacy` attribute holds all privacy-related flags
- `privacy.userTinfoil: true` disables everything (equivalent to setting all flags to false)
- added helper function to core/server/config/index.js to checking privacy flags
- added helper function to core/server/config/index.js to show warning about deprecated items

979c3f237c Prevent loading setup screen if already setup
Closes #3145
- Prevent navigation to the setup screen if Ghost setup
  has previously been completed.
- Fix templates that were incorrectly using foreach instead of each.
- Add validation for minimum password length.
- Fix up functional tests and split out tests for setup to a separate
  instance of casper because setup requires a new database.
- Add a cleanDatabase task to grunt which resets the database to
  new.

1bf975af90 Turn on update notifications for Ember admin
Issue #3160
- Use notifications API to display available update notification.
- Remove update_notification handlebars helper as now both the
  check for an available update and the notification handling
  is run from the server's admin controller index method.
- Bind the notification's location property to a css class
  for styling.
- Refactor Ember notifications to better handle notification
  objects.  Move responsibility for css class generation onto
  the notification component.
- Refactor gh-notifications component to take a location argument
  that's used to assign a css class and filter notifications.

3296a3a41c Flesh out more of the Ember admin
no issue

- this ports over screens from old admin to
 allow people to begin working on aspects of the screen

- All logged out screens have been imported: Signup, Signin,
 Forgotten password, reset password

- Those screens are now ready for behavior to be ported over

- This also updates templates to be more in line with how they were
 in the old admin

- Littered through the code are @TODO comments of functionality that is
 missing and will need to be resolved before this is production ready

- Also scaffolds out the settings screen and every tab

- Configures middleware to provide assets as static files at url `/ghost/ember`

- Modifies default-ember.hbs to use ember option on asset helper
- Remove inline style definition in editor.hbs
- Modifies .gitignore to include ember-hacks.css (in ignored `core/client/assets/css` folder)
