Prevents errors from being uploaded to Sentry when a 404 happens in Ghost Admin. At the moment, 404s in Ghost Admin create an ENOENT error in express' static library. Our generic 404 handler at the end will only intercept requests that don't have any errors in the context, so a simple middleware can strip out 404 errors just before we add in our own.
The Ghost-specific error that we attach to requests does not get uploaded to Sentry :)
- moving this middleware because we're about to add a second piece of middleware
- it's easier to see what we have when each middleware is in its own file rather than in one big middleware.js file
- Reduced our maintenance middleware code down to the bare minimum!
- We have an old maintenance middleware in place to handle when a site is forcibly put into maintenance mode, or the urlService hasn't finished booting
- This maintenance middleware was mounted on every sub app, instead of globally for reasons I no longer remember
- Recently, we introduced a new, static version of maintenence middleware to show during the boot process so we can get the server started earlier & not drop requests
- This version has its own HTML template and doesn't depend on any of Ghost's error rendering code
- To simplify and help with decoupling, this commit merges the two middleware, so that the new independent & static middleware renders its template for any one of the 3 possible maintenance modes
- It only needs to exist in the top level app 🙌
TODO: move the maintenance middleware to its own file/package so it's not part of the app.js as that is weird
- This is a minor bugbare, but it will affect some configuration I'm about to do for c8
- I've been wanting to do it for ages, middleware is plural all on it's own so it's an odd affectation in our codebase
- This also only exists in 2 places, everywhere else we use "middleware"
- Sadly it did result in a lot of churn as I did a full find and replace, but consistency is king!

The only pieces of Ghost-Ignition used in Ghost were debug and
logging. Both of these modules have been superceded by the Framework
monorepo, and all usages of Ignition have now been removed, replaced
with @tryghost/debug and @tryghost/logging.
- Allow the frontend to accept post messages to generate previews of the frontend
- Created a new endpoint in admin we can use to render these previews, which is possibly not necessary
- Supports a limited group of settings, which can easily be expanded, but care should be taken if expanding to use user-provided strings

- Added a wrapper around express.Router to our shared/express util

- Use this shared util everywhre, meaning express is only used directly in this one file

- reduce our usage of express() in favour of Router()
- unify some of our duplicated logic
- fix some structural issues e.g. Sentry
- make it easier to understand the codebase
- added core/shared to watched folders in grunt

- moved express initialisation to a shared file
- always set trust proxy + sentry error handler
- use this new express init everywhere, and remove duplicate trust proxy and sentry error handler code
- we had urlRedirects, urlRedirects.adminRedirect and adminRedirects
- all do kinda similar things, but for different contexts so for now I've done a minimal renaming for clarity
- and updated some comments!!
- also removed totally unnecessary if res.isAdmin clause, as we don't use that, and it was never true

Temporarily reverting whilst we investigate an issue with Sentry and running Ghost via Ghost-CLI.
Ghost-CLI initiated boot was failing when Sentry was installed due to what appears to be `process.cwd()` returning `undefined` here https://github.com/TryGhost/Ignition/blob/master/lib/config/index.js#L26

When using certain proxy setups that result in `host` and `x-forwarded-host` being different, it became impossible to access Ghost because all routes showed generic 404 pages.
- `vhost` module that we are using to separate front-end and admin urls does not use express' `req.hostname` so it does not pick up the `x-forwarded-host` url that express' `'trust proxy'` config gives us
- switched to the forked `@tryghost/vhost-middleware` package which has a one-line change to use `req.hostname || req.host`
- added `'trust proxy'` config to the admin express app and switched to using `req.hostname` in our redirect code to avoid infinite redirect loops

- The refactoring is a substitute for `urlService.utils` used previously throughout the codebase and now extracted into the separate module in Ghost-SDK
