5b30a51adf Enable cycling header levels with ctrl+h. 
closes #3019

* Remove preexisting `ctrl+h` keymap.
* Add logic to cycle the header level.
  * Loops around after h3.
  * Sets cursor position to end of heading
  * Set initial level to h2.
* Remove `ctrl+alt+${NUM}` keymaps (as they do not work
  internationally).

410dc39f3e Add checks for packages and contentPath to startup

- Make sure that require() is able to resolve all dependencies
  listed in package.json.  If packages are missing halt the
  bootstrap process and display an error and help message for user.
- Check that contentPath and its subdirectories exist with the correct
  permissions.
- Check sqlite3 database file is set for read/write access.

- Added server API isInvitation (analog to isSetup), checking if an
invitation exists for a given email address.
- If the invitation is no longer valid (or didn’t exist in the first
place), the user is redirected and an error notification is shown.

- Created "mobile" views: `parent-view`, `content-view` and `index-view`
- `mobile/parent-view` has three callbacks for managing layout, and a mediaQuery listener to keep in sync with the user
- content-view and index-view use their parent-views callbacks to bring themselves into and out of the viewport as appropriate
- fixed media queries for post content list from 800px to 900px
- Created `mobile-index-route` to intelligently transition to a new route on desktops (used by both PostsIndexRoute and SettingsIndexRoute)
- Extract mobile interactions from settings views to new mobile utility views
- `js-` prefixed settings view transitions
- removed unused openEditor action from PostsRoute
- removed unused mobile util "responsiveAction"

I'm not sure why, but @extend's here using placeholder selectors, not classes, work.

This fixes the notification and modal styles, which meant changing how the default button selector works. It is now a placeholder, and a real class extends that. The modal styles extended the default button style, which can now happen.

- Ensure that import operations are run in sequence.
  Previously the operations were started in order but subsequent
  ops were allowed to begin before the previous finished, which would
  result in out-of-order execution.
- Fix bug in attach() where a model property was being passed in
  instead of a transaction object.  If the call was made when a
  transaction was in process, it could cause bookshelf/knex to
  hang and never finish the transaction.

- Before importing, all data is checked for incorrect UUIDs. If check
fails, data is sanitized and outfitted with correct UUIDS.
- Scaffolding allows for easy implementation of additional
sanitization, should that be required in the future.
- Test included, old tests updated.

c02240dd5f Adjust setup and teardown of route tests. 
Closes #3856
- No longer call clearData and initData in test setup because
  migration.init is run during Ghost startup.  The unhandled
  error was a result of clearData removing all the tables while
  Ghost was running--there was a timing issue with the oauth middleware
  checking the refreshtokens table when it had just been deleted.

- Since PostgreSQL handles transactions differently than
  MySQL and SQLite3 there are differences in the way and
  number of errors that are returned.  Update tests to
  only check that at least one error of the proper type
  was returned.

c4fb17d2d7 Reset slug input text box after invalid input. 
No Issue
- When an invalid, all whitespace slug is entered into the
  slug input in the post settings menu, it's rejected but
  the input's value still remains the same.  This resets the
  input back to its original value.
- Added test for the above behavior.
- Only show success notification if slug was actually changed.
- Convert whitespace from tabs to spaces in post-settings-menu.hbs

5ff2a31ce1 Move Models module to have an init method that sets up all models 
resolves #2170

- creates a models.init() function that requires all other model files
and caches them.  This is opposed to the previous functionality where
when you require('./models') it would immediately require all other models.
Now it's done when you want.

- Updates all tests to reflect the new structure of the model module

No Issue
- Defer save until after slug is checked.
- If new slug is empty or all whitespace, reset to previous value.
- If new slug is the same as existing slug except for an increment
  value (e.g. ghost-user-2), use the original slug.
- If the slug has changed, change the URL path to reflect the
  change so that the browser refresh and back button still work.
- Added tests.
