mutation: prefix predecessor hashes with hashing scheme

Summary:
To support future alternate hashing schemes, predecessor identities should be
prefixed with the hashing scheme in use.

Currently there is only one hashing scheme: `hg`, which is the Mercurial
hashing scheme.
