Summary:
Now that all our repos are treemanifest, let's enable the extension by
default in tests. Once we're certain no one needs it in production we'll also
make it the default in core Mercurial.
This diff includes a minor fix in treemanifest to be aware of always-enabled
extensions. It won't matter until we actually add treemanifest to the list of
default enabled extensions, but I caught this while testing things.

Summary:
We may want to use an experimental store for remotefilelog data. This store may
return incorrect data, and cache is going to be poisoned. To prevent it from
hapenning let's add an option that makes it possible to prevent cache
poisoning.

Moves the remotefilelog extension into hgext/ and it's tests into
tests/.
I did not fix up all the check-module errors, since it's a ton of work for
very little impact at this point.
