Several fields are renamed to be consistent with the annotate command, which
doesn't mean the last call for the name unification [1]. Actually, I'd rather
rename line_number to linenumber, linenum, lineno or line, but I want to
port the grep command to formatter first.

I don't have any better name for the list of matched/unmatched texts, so
they are just called as "texts".
As preparation for formatter support, this and the next patch split
linestate.__iter__() into two functions, line scanner and displayer.
New code uses regexp.search(str, pos) in place of regexp.search(substr),
which appears to fix a bug of highlighting.
If p1 is null, ':.' is translated as '0:null'. But rangeset can't handle null,
only revision 0 was visible. Because 'null' should not be listed implicitly,
"log --follow" (without -r) should be empty if p1 is null.
Test of "hg grep -f" is added for cmdutil.walkchangerevs().
'*' causes the resulting RE to match 0 or more repetitions of the preceding RE:
>>> bool(re.search('.*', ''))
>>> True
This causes an infinite loop because currently we're only checking if there was
a match without looking at where we are in the searched string.
Some tests ended up in a directory several directories deeper than $TESTTMP,
usually because some 'cd ..' had been forgotten between different test cases.
Add 'cd ..' where they are missing so the tests get back where they started.
Matching lines without trailing '\n' was missing the last character.
That seems to have been an unintended side effect of 8abe3f27975c.
The test in c21748e4cd4d documents the bad behaviour.
