Parent will now always be updated or added when qrefreshing HG patches. Plain
patches will not be changed, but patches that neither are plain nor HG will be
upgraded to HG patches on first refresh.
Do not pass reject file content to patchfile.writelines() to:
- Avoid line endings transformations
- Avoid polluting overriding implementations with unrelated data. They should
override write_rej() to deal or ignore reject files properly.
