Added a .git-blame-ignore-revs file

I've added a file `.git-blame-ignore-revs`, containing the rev of the
codebase-wide ormolu reformatting.

Issuing the command `git config blame.ignoreRevsFile .git-blame-ignore-revs`
causes `git blame` to 'see through' the renaming as best it can.
