Samir Talwar f9ff7ae00c CI: Remove the custom Microsoft SQL Server image. 
Now that we use the mssql-tools image for running `sqlcmd`, we no longer need this (or indeed use it very much).

TODO(swann): write this out better

Any tests in test-server.sh should also have their names added to server-test-names.txt, otherwise the CI pipelines will not run them.
