Show 5-hour and 7-day Claude limits in menu bar

The menu bar title now shows both Claude usage windows with emoji prefixes: ๐Ÿ• for the 5-hour window, ๐Ÿ“… for the 7-day window. Windows at 0% or absent are omitted from the title. The popover's hero number is now the 5-hour window (matching the menu bar), with 7-day and 7-day Opus as supporting bars below. Removed the old claudeHeadline system that picked a single binding limit โ€” the menu bar now shows all windows directly.
